Case Management for Long Term Care

Community Care Program for the Elderly and Disabled (CCPED)

Through the CCPED program, home and community-based services are provided for medically and financially eligible persons who are 65 years or older or for persons of any age who are disabled.

The assigned case manager is a strong advocate for the patient and is responsible for assessment of need, placement and monitoring of services.

Services can include nursing care, medical day care, medical transportation, respite care, adult day care, and homemaker services.

Community Resources for People with Disabilities Waiver (CRPD)

CRPD is designed to provide services in addition to full Medicaid benefits to individuals who would otherwise be unable to remain in their community. Services include case management, environmental/vehicle modifications, community transitional services and personal emergency response systems.

Medical and financial criteria must be met to be eligible.

Traumatic Brain Injury (TBI) Waiver

The TBI Waiver helps people between the ages of 21 and 64 who have survived a traumatic brain injury after the age of 16. Services include case management, structured day program, personal care, community residential services, transportation, respite care and cognitive therapy.

Medical and financial criteria must be met to be eligible.

Traumatic Brain Injury (TBI) Fund

The TBI fund enables New Jersey residents who have survived a brain injury to receive the services and support they need to live in their community. Funds are used where insurance, personal resources and /or public programs are unavailable to meet the needs of the individual.
