Monthly On-Site Visits

Many adults are able to live independently in the comfort of their homes or place of residence as they grow older but can benefit from occasional visits by a certified healthcare professional to ensure they are doing everything possible to maintain their healthy lifestyle.

Depending on your needs, VNACJ nurses can provide care on a one-to-one basis or offer health care education to groups at community centers, assisted living, senior centers, soup kitchens, and faith-based centers.

Medication Review and Instruction

FREE Health Screenings and Blood Pressure Monitoring

Health check-ups should be a regular part of every health maintenance program, especially if you have diabetes, high blood pressure, or concerns about cholesterol or hypertension.

At VNACJ, we offer FREE health screenings and blood pressure monitoring at neighborhood locations including senior housing, senior clubs, and nutrition sites. Screenings are also available for community businesses.

Physical and Psychosocial Assessments

To stay your healthiest and enjoy life to the fullest, it’s important to have regular health assessments.

Through our Senior Wellness program, a registered nurse, social worker or therapist will visit community sites such as senior housing, senior clubs, nutrition sites, or senior centers and perform a complete health assessment. Following the assessment, we can send a follow-up report to you, your healthcare provider, or responsible family members to make sure you maintain your current level of health and well-being.

One-on-One Counseling

One-on-one counseling for disease prevention and control of chronic conditions is available through our Senior Wellness program to assist you in your care management needs. In addition, an in-home evaluation visit can be made for referral to in-home services or for assistance with alternative community resource services.

Group Health Education

VNACJ is committed to the promotion of health education and wellness, especially for our seniors. Our healthcare professionals make presentations on a wide range of important health issues such as hypertension, nutrition, diabetes, cholesterol, osteoporosis, and life style management.
