This referral phone line is a service of Monmouth-Ocean HIV Care Consortium and community organizations in Monmouth and Ocean counties. Assistance is offered daily from 8:30am-4:30pm. After-hours messages will be returned on the next business day. All calls are confidential.
As the lead Planning and Administration agency for the Monmouth-Ocean HIV Consortium, our mission is to identify, prioritize and provide services to eligible clients with HIV or AIDS. Programs and services include:

HIV/AIDS Outreach Programs
- Prevention Resource Network (PRN)

A federally-funded community outreach program developed to prevent/reduce the spread of HIV infection. One-on-one prevention case management services are available to all individuals who are at risk for HIV and including those who are HIV+.
- The Life Improving Network to the Community (LINC) program
Provides AIDS outreach to the
Monmouth County Correctional Institution and includes case management services in and out of facility,
as well as peer education and discharge planning prior to parole or discharge. Community follow-up
continues for 6 months from the date of discharge or parole.
- Intensive outreach to new or untreated individuals (ICON)

The ICON program is designed to reach out to individuals infected with HIV in the community who have never received treatment for the disease or have not maintained treatment. Through counseling and education, we strive to enroll individuals into an ongoing treatment plan. The program is available to residents of Monmouth County.
- HIV Treatment Adherence Program

All clients identified as HIV+ can access the treatment adherence program for education regarding their medication regimen and identification of additional medical needs. The program is funded by a Ryan White Title II grant.
- Ryan White Community Based Management

As part of the Ryan White Community Based Management program, VNACJ case managers help individuals infected with HIV to access care and treatment services. Services can include, but are not limited to, primary medical care, medication assistance, substance abuse treatment, emergency financial assistance and housing. In addition, case managers have access to many other community-based resources including HIV Home Care and ACCAP.
- Street and Community Outreach for Positive Education Progams (SCOPE)

Through a grant funded by the New Jersey AIDS Partnership, VNACJ staff provides outreach to Asbury Park and surrounding towns. Tent events are held in various locations through the community and offer screening, testing and local entertainment in an informal, relaxed atmosphere. HIV testing and one-on-one counseling are available and confidential.
- AIDS Community Care Alternatives Program (ACCAP)
ACCAP is a special waiver program for people of any age with AIDS and children up to age 13 who are HIV positive. VNACJ case managers work with qualified people for assistance with private-duty nursing, medical day care, personal care assistance, and hospice care.

Buddy Program
Through the Buddy Program, volunteers are recruited to provide companionship, support and assistance for Hunterdon County clients infected with HIV/AIDS. May also include respite care upon request, as well as referrals to other possible support services.

Prevention with Positives
A prevention case management service provided to individuals who are HIV positive. Program offers one-on-one case management, health education and risk reduction group sessions.
