Hospice Services
Through the use of an interdisciplinary team approach that is always directed by the patient and family, our services can be provided in the patient's home or place of residence (i.e., nursing homes and assisted living) and include:
- Patient and Family Centered Care
Care is directed by the patient and family through a holistic, compassionate approach.
- Physician coordination
A VNACJ hospice medical director or physician acts as a consultant and resource, working closely with patient's attending physician.
- Nursing Care
A registered nurse is on hand to provide nursing care, order medical equipment, monitor symptoms, medication, and physical changes. In addition, they will teach the family how best to provide care and support.
- Social Services
A licensed social worker works with the family and patient to provide emotional support, advocacy and access to community resources at this crucial turning point.
- Home Health Aides
An integral part of the hospice team. Certified home health aides provide personal care such as bathing, grooming and companionship. They can also perform light housekeeping, meal preparation and run errands.
- Physical, Occupational, Speech Therapists
Experienced hospice therapists work with the patient to maximize patient comfort.
- Complementary therapies including pet, music and massage therapies. Reiki is also available.
- Spiritual Support and Counseling
A member of the pastoral care team is available to pay regular visits to patient and family.
- Hospice Volunteers
A specially trained group of volunteers lend their compassionate support and time to listen, read, or assist with everyday tasks and errands.
- Bereavement Support
Following the loss of your loved one, our staff continues to support surviving family members throughout the following year as they journey through the grieving process.
Support may include family or individual counseling or support group referral. On-going bereavement support and special children and adult activities are available to the community that include annual memorial service, hearts of remembrance children's day and hospice tree lighting.
