Joint Replacement Rehabilitation

Are you having hip, knee or joint replacement surgery soon? In-home post-operative rehab services can be arranged prior to surgery for a smooth and easy transition from the hospital. One-on-one therapy can begin within 24 hours after discharge and typically lasts only 3-4 weeks.

VNACJ Joint Replacement Services are specially designed for patients undergoing orthopedic surgery. Falls Prevention Program

Developed especially for patients who have experienced falls recently or in the past, information is placed in the patient's record to notify each member of your VNACJ home care team who is providing care. In this way, your nurse, therapist or home health aide understands that falling can be an issue for you and will be sure to include fall prevention steps in their routine plan of care, including:
  • preventive education
  • development of safety measures in the home to prevent falling
  • identification and evaluation of any home safety concern

Home Safety and Medical Equipment Evaluations

A licensed physical therapist will come to your home and perform a complete home evaluation to address your needs. Evaluations vary but can include:
  • Ability to function in home
  • Safety issues both inside and outside the home
  • Condition of medical equipment and education on appropriate use
