Our Social Workers are an important part of the VNACJ home care team. They consult with other team members regarding patient problems, interface with patients and their families and even facilitate support groups for families of the terminally ill. No one should have to deal with important health care issues on their own.

Services include:

• Mental and psychosocial health care
• Therapeutic counseling and support
• Health care liaison — strong patient/family advocate
• Knowledgeable resource for alternative community support programs patients and families need most
• Bereavement support
